The optimal extent of resection of duodenal adenocarcinoma is not clear. Some authors recommend PD for all patients with duodenal adenocarcinoma, including patients with D3 and D4, to ensure adequate total resection. 13,14 The value of conventional extensive resection of lymph node dissection has recently been challenged 5. PD was used for proximal duodenal cancer but partial excision of D3 and D4.5, 9, 10, 15 cancer did not adversely affect local resection. This may not be unexpected as most of the lymphatic vessels in these distal parts of the duodenum are excreted into small mesentery rather than pancreatic duodenal lymphatics removed by PD. In a series of studies there were no significant differences in 5-year survival between patients who received radical resection (52%) and patients who underwent partial resection (61%).
