Nursing Essay: Patient Assessment

This work is done based on the preliminary assessment process that the patient experiences when it arrives at the endoscopic examination unit. This was placed in the endoscopic examination unit in the second year of adult diploma treatment. I will explore the patient's overall needs, determine the focus of care that the patient needs, then I emphasize certain priorities and give rationality behind it. During my hospitalization I completed under the supervision of a tutor and I am evaluating that a 37 - year - old woman has arrived at the unit for upper gastrointestinal endoscopy. During the preliminary evaluation period, it is important that each patient is an individual who needs unique care, so it is important to conduct an overall evaluation.

According to NMC 2008, nurses stated that they must protect and promote dignity, and all preliminary evaluation is done in a private room (NMC 2008). In front of me, my leader and reception staff knew that patients had learning disabilities. I try to use short sentences and appropriate words. I also make sure that there is time to deal with what the patient told her.

I think that the patient understands the procedure during the evaluation period, but I am worried about worrying about all possible complications and postoperative procedures. Instructions are well understood, and I am worried about the patient's ability to express any doubts that she may have and patients also have high blood pressure. The focus of the patient is to monitor the blood pressure of the whole unit, the ability to understand procedures and consent, and the communication barriers associated with learning disabilities. Because it is indispensable that the patient has a complete understanding of the surgical process, this patient's most important priority is her learning disability and communication barriers.

Nursing assessment is the collection of information on the physical, psychological, social and psychological state of patients by authorized registered nurses. Nursing assessment is the first step in the care process. Part of the care assessment can be left to the certified nurse assistant. Vitals and EKG may be licensed to certified nurse assistant or nursing technician. (Nurse Magazine, 2017) It is different from medical diagnosis. In some cases the scope of care assessment is very broad, but in other cases care assessment may be focused on physical system or mental health. Care assessment is used to determine current and future patient care needs. It combines understanding of normal and abnormal physical physiology. By identifying and prioritizing appropriate interventions, you can identify appropriate changes and critical thinking skills in a timely manner. Evaluation formats may have been developed for use in certain facilities and in certain circumstances.

The health examination care class is one of the most important courses for students to attend nursing schools. Nurses have many responsibilities, such as evaluating patient needs. The nurse must comply with the evaluation tool to determine the treatment policy of each patient. Initial health assessment is important for the patient's ongoing health. Nursing students participating in nursing school health checkup programs can provide a basis for the daily work of nurses. Proper documentation of health assessment information is important for patient care.
