You can also fill out a health insurance application under the "Application and Form" menu in dwss.nv.gov and mail it to the address listed on the form.

If you are interested in applying for a qualified health plan, please apply at healthcare.gov. When the account creation screen appears, select [Nevada] from the status drop-down menu and start the application process of [Qualified Health Plan].
