My life is the window to your electronic medical record. This is part of Lifespan's electronic health record system LifeChart, which allows you to create and manage a single record for each patient. This record contains information on all lifetime providers.

All your health information is maintained and shared according to applicable federal and state laws and strictly conforms to HIPAA rules.
To ensure the security of your health information, there are two ways to register: by requesting your personal access code from your Lifespan provider or by answering some personal questions Apply for personal access code online.
If you want to receive your personal password online, your identity will be verified by Experian, a world leading information service company. Fill out the safety form, answer all questions, check and verify your identity through Experian. Apply for codes online now ยป
After registering, please visit Lifespan.org / MyLifespan and click [Login] to access health information. You can manage your health information using the tabs at the top of the screen.
