Awareness Questionnaire (AQ) consists of Mark Sherer, Corwin Boake, Ellen Levin, Bernard V. Silver, George Ringholz, Walter M. High, Jr. First version of development. The result of AQ and factor analysis led to the current version of AQ announced in 1998. AQ is an indicator of self-cognitive impairment following traumatic brain injury (TBI). At the moment, all AQ patients are studying TBI patients, but AQ may be suitable for other types of acquired brain injured patients such as cerebral hemorrhage, brain tumor, hypoxia and so on.

AQ consists of three forms, one form completed by TBI patients, one completed by family / important others and the other completed by clinicians familiar with TBI patients. 17 self-assessment and family / important forms, and 18 clinicians. In this survey, we studied the consciousness and motivation of UTM students towards organ donation. The raw data was collected by randomly distributing the questionnaire to 60 students.
