An abnormal left coronary artery (ALCAPA) from the pulmonary artery is a congenital (existing at birth) cardiac defect in which the left coronary artery abnormally exists from the pulmonary artery. Usually, the left coronary artery and the right coronary artery are from the aorta and supply blood to the heart.

In children with ALCAPA, the left coronary artery is derived from the pulmonary artery and delivers blood to the left side of the heart without oxygen.
