3. Forces of Change Assessment (FOC)

Change intensity assessment is designed to help MAPP participants answer the following questions: "What are the factors that are affecting the health of our community and regional public health systems?"

At this stage, participants will participate in brainstorming sessions to identify trends, factors, or events that affect the health and quality of life of community and local public health systems. The strengths identified through this process and the results of the other three MAPP evaluations serve as the basis for the next MAPP phase - identification of strategic problems
